Gentiva Hospice Foundation's mission is to support and educate persons and organizations dealing with life-limiting illnesses.
The Foundation is a 501(c)(3) nonprofit organization funded by generous donations from grateful families and friends of our patients.
Our Foundation is committed to support deserving organizations and causes associated with hospice and palliative care and giving compassionate assistance to those at the end of life's journey.
History of the Foundation
In 1996, Odyssey HealthCare, Inc., launched its first Medicare-certified hospice program in Indianapolis, IN. The Odyssey HealthCare Foundation was formed in May 1998 as a vehicle to manage donations from grateful families and friends of our patients. In March 2008, Odyssey HealthCare, Inc. acquired VistaCare, Inc. Odyssey HealthCare Foundation changed its name to Odyssey VistaCare Hospice Foundation in October 2008 as it assumed the additional role of servicing and supporting all VistaCare sites and their patients. In August 2010, Odyssey HealthCare, Inc. was acquired by Gentiva Health Services of Atlanta. As a result, the name of the foundation was changed in January 2012 to Gentiva Hospice Foundation. The Foundation additionally supports Gentiva hospice locations and their patients.
